Code of Ethics

I, __________________________________ , understand the high level of commitment
involved in participating in pet-assisted therapy through Furry Ministries. I realize
that the people my pet and I will serve through Furry Ministries depend on us to
give our very best while visiting. I agree to participate in a reliable and responsible
manner and adhere to the following items below:

Honor visitation commitments – be on time and keep all scheduled visitations
unless there is a true emergency. The minimum number is at least 4 visits per
year.
Always contact the Furry Ministries coordinator if you can’t make a visit - this
contact should be made as soon as you know you can’t make a visit.
Arrive prepared for every visit – have appropriate leash, water and bowl for
your dog, treats (if used), and other items such as a dribble towel to wipe the
dog's mouth after a drink.
Annual health checkups – ensure your dog has his/her annual health check-up
and paperwork is updated with Furry Ministries.
Appropriate grooming – for both you and your dog.
Patient confidentiality – do not discuss with others information obtained about
patients during your visits or anything a patient may reveal to you.
Visit only when you and pet are healthy – ensure you and your pet are well
when visiting.
Abide by TDInc guidelines and requirements – become familiar with these
items and follow them all.
Adhere to the rules and regulations of each facility – each facility may have its
own requirements. Be sure to follow them as well.
Do not provide any kind of assistance to residents – do not even provide a
drink of water. Instead call a nurse.
Clean up after dog – never leave a trace your dog was there.
